Please "click" the above links to access the Water System Service Line Questionnaire. You can use the fillable form and e-mail to davidward@ogden-ks.gov or use the online survey
You may also pick up the questionnaire at the Ogden Community Center, Ogden City Hall, and the Ogden Post Office. Please fill out the questionnaire and return it to the City of Ogden located @ 222 Riley Avenue by MAY 1st 2024.
